JOB SUMMARY
The primary responsibility of the Physical Therapist (PT) is to plan, delegate and provide safe and competent physical therapy interventions as ordered by physician.
ESSENTIAL FUNCTIONS
* Provide physical therapy assessments, as well as develop and implement individualized treatment plans to increase client's independence and functioning
* Supervise Physical Therapy Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards
* Identify need to make referrals to other disciplines to address comprehensive care needs of the client
* Demonstrate competent technical skills according to current Physical Therapy standards of practice
* Communicate changes in resident's condition
* Participate in discharge and community mobility planning
* Prescribe necessary durable medical equipment to facilitate independence
* Provide accurate and timely billing records and supervise supporting clinical documentation
* Respond to family requests and concerns timely
* Assure resident abuse or neglect does not occur
* Observe confidentiality and safeguards for all patient information
* Participate in collaborative care conferences
* Coordinate and provide training with Restorative Nursing and other Care staff
* Provide education and training to patient, caregiver, and community partners
* Assist with training and education of facility and professional partners
* Ensure compliance with mandatory education, training and competency programs
* Performs other duties as assigned
JOB QUALIFICATIONS
* Education
* Graduate of an accredited physical therapy school.
* License
* Current Wyoming Physical Therapy License
* Certifications required
* See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
* Experience
* Experience in providing appropriate care in post-acute or long-term care environment preferred
Knowledge, Skills, and Abilities:
* The ability to maintain continuing education requirements and all certifications and licenses as required to maintain professional license.
* Licensed professionals must strictly adhere to the ethical code of their respective discipline.
* Must use good judgment in interpreting patient referrals and performing appropriate physical therapy procedures.
* Ability to develop positive interaction with patients, patient families, physicians, and staff in order to effectively care for the patients.
* Must be able to assess a patient's condition, formulate a plan of care, select appropriate interventions, and evaluate patient response to care/treatment and to explain/teach patients about their condition/recovery.
* Ability to perform multiple tasks simultaneously with frequent interruptions.
* Analytical and reasoning skills with a high level of mental concentration and memory.
* Ability to speak clearly to communicate and interact positively with patients, patient families, physicians, and staff in order to effectively care for the patients.
* Computer Knowledge including the ability to enter material into electronic record. Ability to operate fax, computer, and copy machine to perform the duties of the position.
* The ability to demonstrate knowledge of developmental stages and apply developmental theories/concepts when planning and implementing care for the appropriate age of the patient. Ability to maintain the appropriate level of knowledge and skills.
